Key Clinical Formulas
ฮP in Obstructive Sleep Apnea: Critical closing pressure (Pcrit)
Discrimination Score: % correct on monosyllabic word list at 40 dB SL
Conductive hearing loss: outer/middle middle ear problem (otosclerosis, effusion, perforation). Sensorineural: cochlea or CN VIII (noise exposure, presbycusis, Meniere's). Mixed: both. Audiogram + tympanometry + ABR for diagnosis.
Acute bacterial rhinosinusitis: symptoms >10 days, severe onset, or double-sailing. Chronic: >12 weeks with nasal obstruction, discharge, facial pain, anosmia. Treatment: saline irrigation, intranasal steroids, antibiotics (amoxicillin-clavulanate).
Squamous cell carcinoma most common. Risk: tobacco, alcohol, HPV (oropharynx), EBV (nasopharynx). Presentation: persistent ulcer, mass, dysphagia, hoarseness >2 weeks, unilateral ear pain. Diagnosis: panendoscopy + biopsy, imaging (CT/MRI/PET).
Adenotonsillar hypertrophy: obstructive sleep apnea, failure to thrive, cor pulmonale. Tonsillectomy indications: โฅ7 episodes/year, โฅ5/year for 2 years, โฅ3/year for 3 years. Otitis media: most common childhood infection โ effusion >3 months โ tympanostomy tubes.
Vocal fold nodules: voice abuse. Vocal fold paresis: recurrent laryngeal nerve injury. Laryngeal cancer: hoarseness, dysphagia, stridor, neck mass. Treatment: radiation, partial/total laryngectomy, voice rehabilitation.

Worked Examples
Air conduction thresholds: 250 Hz=20 dB, 500 Hz=25 dB, 1000 Hz=30 dB, 2000 Hz=35 dB, 4000 Hz=40 dB
PTA = (25+30+35+40)/4 = 32.5 dB HL โ mild hearing loss
Child with tonsillitis: 8 episodes in past year, 6 in year before. Meets criteria: โฅ7 episodes in 1 year โ tonsillectomy indicated.
Bone conduction normal (0-15 dB), air conduction 50 dB with 30 dB gap โ conductive hearing loss. If air and bone both at 50 dB โ sensorineural. If bone 25 dB and air 50 dB โ mixed.
Practice Problems
Answer: โฅ7 episodes of tonsillitis in the past year, OR โฅ5 episodes per year for 2 years, OR โฅ3 episodes per year for 3 years. Also indicated for OSA and peritonsillar abscess.
Answer: Conductive: normal bone conduction, elevated air conduction, air-bone gap >10 dB. Sensorineural: both air and bone equally elevated, no air-bone gap. Mixed: both patterns combined.
Answer: Adenotonsillectomy โ curative in 80-90% of otherwise healthy children with OSA.
Answer: Aspirin sensitivity + nasal polyps + asthma. Patients must avoid NSAIDs. Treatment includes aspirin desensitization, biologics (omalizumab, dupilumab), and surgery.
Answer: I Olfactory, II Optic, III Oculomotor, IV Trochlear, V Trigeminal, VI Abducens, VII Facial, VIII Vestibulocochlear, IX Glossopharyngeal, X Vagus, XI Accessory, XII Hypoglossal.
